 Important:

2. It is not mandatory, but we recommend, that the person who is filling this request is the same person that is conducting the FlexReport/survey assessment.

- We believe FlexReport survey assessment is not a judgmental assessment evaluation survey, but rather a reminder to organizations in regards to health. The FlexReport will allow making choices, and options towards businesses best practices. This is an opportunity for everyone to make healthier choices as part of a Culture of Health in Business.  We believe organizations of all shapes and sizes can make an enormous difference for themselves and for everyone.
- CHBO, the designer of this survey is solely responsible for its hosting and not its digital security. Your participation and the results to the FlexReport survey assessment is a paid voluntary and/or on your request for your organization. If you are asked in the survey to reveal your identity or the identity of the organization on whose behalf you are responding and do not wish to do so, please do not participate in the FlexReport assessment survey. Your response will be used by CHBO only for the purpose of assessment in relation to the Culture of Health in Business.
- If you are given an opportunity to submit responses to this assessment survey, please provide relevant and responsible responses; your response will be processed by CHBO for the purpose of the Culture of Health in Business in relation to your organization; if you do not wish your response and the information it contains to be processed by CHBO, please do not participate and respond to the survey..
- Do not forward to a third party this survey without the permission of CHBO, and please contribute to its effectiveness by responding only once every twelve months in regards to your organization (not your employees).
- CHBO (or any CHBO information/content) and our FlexReport survey assessment are provided by CHBO on an 'as is' and 'as available' basis. CHBO does not warrant that FlexReport survey assessment or E-Mail sent, including this survey content, are free of viruses or other harmful components. CHBO will do its best to protect all aspects of the FlexReport survey assessment and/or CHBO website, but will not be liable for any damages of any kind arising from this survey assessment content, including, but not limited to direct, indirect, incidental, punitive, and consequential damages.
- CHBO reserves the right to terminate or withdraw a FlexReport survey assessment, and your opportunity to participate in a FlexReport, at any time and for any reason. CHBO alone has the right to view and use the survey results and disclose the survey assessment results to you only.
CONFIDENTIALITY: Your responses to the FlexReport will be kept strictly confidential. The online FlexReport information collection tool and automated system and the hosting-server are maintained by third-party vendors, under the supervision of CHBO. CHBO/FlexReport information collection will be used for assessment and evaluations towards your organization in relation to the Culture of Health in Business only. Any use of your individually identifiable information for business relation or other purposes will require your organization prior written consent. This disclaimer will abide by CHBO Code of Ethics.

CHBO is a not-for-profit organization, You and/or Your Organizations are requesting CHBO to conduct a paid service in order to receive Certification from CHBO, once you submit this information; we will then contact you, to proceed with the payment.  Once payment received, we will send your private access token --to start your FlexReport Assessment Survey.  Payment entity must match the name of the - organizations name- that you provided here in this form. If you have any question, please visit our question page.

Once payment is received, they will be no refund. Within 24 hours, we will send your private password-protected link to start the FlexReport assessment-survey.
